What Is Gender Dysphoria?
Gender dysphoria is the distress a person feels when their gender identity differs from the sex they were assigned at birth. It is a recognized clinical condition, and it can cause significant emotional and social strain, well beyond simple discomfort. Deogracias et al. (2007) built the GIDYQ-AA to measure it on a continuous scale, from fully identifying with one's assigned sex at birth to fully identifying with another gender. Unlike a simple yes-or-no diagnosis, it captures where you fall on that spectrum.
What It Measures
The questionnaire covers four areas of gender experience over the past 12 months:
- Subjective feelings — how you feel inside about your gender identity
- Social experiences — how others treat you and what roles you feel pressured into
- Physical/body feelings — discomfort with your body and medical transition wishes
- Legal/social identity — legal name or gender marker changes
Scoring
The official scoring takes the average across all 27 items, from 1.0 to 5.0. Lower scores mean more gender dysphoria, and the original study set the cut-point at 3.0:
- 3.0 or below — suggests clinically significant gender dysphoria
- Above 3.0 — less likely to meet the threshold for a clinical concern
The scale showed excellent internal consistency (alpha = .97) and strong ability to distinguish between clinical and non-clinical groups in the original validation study.
