What Is Gender Dysphoria?

Gender dysphoria is the distress a person feels when their gender identity differs from the sex they were assigned at birth. Gender dysphoria is a recognized clinical condition. It can cause significant emotional and social strain, well beyond simple discomfort. (2007) to measure this on a continuous scale, from fully identifying with one's assigned sex at birth to fully identifying with another gender. Unlike a simple yes-or-no diagnosis, it captures where you fall on that spectrum.

What It Measures

The questionnaire covers four areas of gender experience over the past 12 months:

  • Subjective feelings — how you feel inside about your gender identity
  • Social experiences — how others treat you and what roles you feel pressured into
  • Physical/body feelings — discomfort with your body and medical transition wishes
  • Legal/social identity — legal name or gender marker changes
There are 27 items, each rated on a 5-point scale. It takes about 10 minutes to complete.

Scoring

Your final score is the average of all 27 items (some reverse-scored). Scores range from 1.0 to 5.0:

  • Below 3.0 — suggests significant gender dysphoria; this is the clinical cutoff
  • 3.0 and above — less likely to meet the threshold for a clinical concern
The scale showed excellent internal consistency (alpha = .97) and strong ability to distinguish between clinical and non-clinical groups in the original validation study.
