Do you have thoughts you cannot shake, or urges to check, clean, or arrange things until they feel "right"? The OCI-R covers six types of OCD symptoms in 18 items — washing, checking, obsessing, mental neutralizing, ordering, and hoarding. About 5-8 minutes.
It started as a 42-item inventory. Foa and colleagues trimmed it to 18 in 2002 because the shorter version turned out to work just as well for screening and tracking treatment. It is one of the more common tools in the area, mostly because it is short enough to use repeatedly.
Domains
- Washing — excessive washing and cleaning
- Checking — repeated checking behaviors
- Obsessing — distressing intrusive thoughts
- Mental Neutralizing — mental rituals
- Ordering — arranging and symmetry needs
- Hoarding — difficulty discarding items
Scoring
5-point Likert (0=Not at all, 4=Extremely). Total score range 0-72. Subscale scores 0-12 each. Cutoff: total >= 21 suggests clinically significant OCD symptoms.
Psychometrics
Internal consistency alpha = .81-.93 per subscale. Test-retest reliability = .74-.91. Total >= 21 has sensitivity 0.84, specificity 0.79 for OCD diagnosis.
Note: The OCI-R is an assessment tool. It cannot replace a clinical diagnosis.
Scoring Guide
Total score range 0-72. Six subscales (Washing/Checking/Obsessing/Neutralizing/Ordering/Hoarding) each range 0-12. Cutoff >= 21 suggests clinically significant OCD. Cronbach alpha = .81-.93 per subscale. Good discriminant validity for OCD.Result Interpretation
