What Is Prolonged Grief Disorder?

Prolonged grief disorder (PGD) is a specific reaction to the loss of someone loved very much. It involves persistent longing, emotional pain, and difficulty moving forward that lasts beyond 6 months and significantly impairs daily functioning.

Scale Structure

The PG-13 (Prigerson & Maciejewski) contains 13 items screening for PGD criteria:

  • Event criterion (assumed)
  • Separation distress (items 1-2)
  • Duration criterion (item 3)
  • Cognitive, emotional, and behavioral symptoms (items 4-12)
  • Functional impairment (item 13)

Scoring

Items 1-5 and 13: scored 0/1 or 1-5. Items 6-12: scored 1-5. Higher scores indicate more severe grief symptoms.

Clinical Cutoff

Symptom items (4-12) score ≥30 suggests syndromal-level PGD (Prigerson et al., 2008).
